Enhanced Form R
Form R Part B is only available through Synapse. To generate your Form R, log into Synapse, and on your homepage you should see a link in the left hand corner saying 'Form R'. Click on this, make sure your information is up-to-date, and then generate your Form R. Please note that previously, we required that the Form R have a physical signature. There is now a button to click on that acts as an electronic signature. Once this is complete you must generate the form as a pdf, and then attach it to your e-Portfolio.
Please note that if your enhanced Form R is not uploaded to the e-Portfolio, your outcome will be withheld. If this ARCP meeting is the final review ahead of your CCT/CESR date, failure to submit a completed enhanced Form R is likely to result in a delay to your CCT/CESR application.

Up-to-date RCoA Log Book summary for year being reviewed, including summaries for each module done in the training year under review - to be uploaded to e-Portfolio.

Up-to-date Curriculum Vitae – ensure this is uploaded to your e-Portfolio and bring ONE copy with you to your ARCP. This should include a summary of the following:
- Internal teaching attended
- External courses attended – attendance certificates uploaded to e-portfolio
- Attendance at Lead Provider training days
- Audits completed – with evidence added to your e-portfolio and summary of your involvement
- Presentations given – with evidence added to your e-portfolio
- Research – with evidence added to your e-portfolio
- Publications – with evidence added to your e-portfolio

Evidence of safeguarding children training - Level 2- upload certificate to your e-portfolio.

Evidence of safeguarding vulnerable adults training - Level 2 - upload certificate to your e-portfolio.
[NB Safeguarding Children and Vulnerable Adults are usually covered at Trust Induction. If you are unable to provide evidence of such, then both can be done up to Level 2 online at e-Learning Anaesthesia (eLA).]

Academic Clinical Fellow/ Clinical Lecturers Only – An Academic Supervisor’s Report
The following must be provided within RCoA E-portfolio:

Educational Supervisor’s Structured Report (ESSR) – one to be completed, on e-portfolio, at the end of the year to inform the ARCP panel.

Interim Progress Report(s) (IPR) - one completed, on e-portfolio at the end of each placement / hospital / module EXCEPT at the end of the year, prior to your ARCP, when an ESSR is completed.

Unit of Training Sign-off for EACH completed key unit AND general sub-units of training.

Anaesthetic Clinical Evaluation Exercise (Anaes-CEX) – completed since last review
[Minimum of 1 successful outcome per unit of training completed]

Directly Observed Procedural Skills (DOPS) – completed since last review
[Minimum of 1 successful outcome per unit of training completed]

Case Based Discussion (CBD) – completed since last review
[Minimum of 1 successful outcome per unit of training completed]

Anaesthesia List Management Assessment Tool (ALMAT) –
CT/CT2 (Basic) - Minimum of 1 successful outcome for General Duties and Urological and Gynaecological Surgery
ST3/4 (Intermediate) - Assessment requirements for General Duties: Option of A-CEX or ALMAT for each unit blueprinted with both types of assessment. Minimum of 3 CBD. Airway course with assessment can replace DOPS for Airway Management.
ST5/6 (Higher) - Assessment requirement for General Duties: Option of A-CEX or ALMAT for each unit blueprinted with both types of assessment. Minimum of 3 CBD.
ST7 – Minimum of 1 successful outcome for General Duties (except airway management, sedation and conscious sedation for dentistry; trauma and stabilisation; transfer)

Intensive Care Medicine-Acute Care Assessment Tool (ICM-ACAT)
[Minimum of 1 successful outcome per unit of training completed]

Multi-Source Feedback (MSF) – one for each training year

Evidence of Completion of Generic Skills (Advanced Training Domains 1 - 6) – (Higher & Advanced trainees Only)
